The City is partnering with BlueCross BlueShield and Jewel-Osco to fight the flu by hosting a flu vaccination event in the Evanston Township High School parking lot, located east of Dodge Avenue and north of Lake Street, on Sunday, Nov. 29 from 10 a.m. to noon.

The event is for Evanston residents ages three years and older, and is open to both drive-through and walk-through participants. High dose vaccines are available for elderly residents. Due to a limited number of available vaccines, community members must register in advance for this event at cityofevanston.org/flu by Thursday, November 26. 

Participants should bring an ID and their insurance card with them to the event, or be able to provide the name of their insurer. Those without health insurance are welcome. Participants who do not have insurance will not be billed or charged. Participants should also download and fill out this form in advance and bring it with them to the event. Each individual family member is required to submit their own registration form. Masks must be worn at all times.
